MR1: The Category One discount percentage of the FCC Form 471 was modified from 0% to 50% as per applicant documentation. <><><><><> MR2: According to the FCC Order 14-99, Voice services are subject to a 20% phase down of support beginning in funding year 2015. As a result, the Discount Rate of this FRN was reduced from the Category One Discount rate of 50% to 30%. <><><><><> MR3: Based on the applicant's request, the One-time Eligible Cost of the FRN's Item 21 was changed from $1,550.00 to $742.14. <><><><><> MR4: The amount of the funding request was changed from $742.14 to $449.73 to remove the ineligible products/services: Equipment Purchase- $262.47 and Total Equipment Coverage-$29.97.
