DR1: The Form 470 cited does not include the billed entity or the entities receiving service on the submitted Form 471. <><><><><> DR2: Applicant has not provided sufficient documentation to determine the eligibility of this item. No documentation was provided for item(s): make/model of equipment. <><><><><> DR3: FCC rules require that a fair and open competitive bidding process occurs. USAC attempted to ensure that a fair and open competitive bid process occurred regarding FRNs awarded to service provider LynxStar. USAC made multiple attempts to contact consultant Blanca Ramos and LynxStar regarding this issue; however, USAC was unable to make successful contact. USAC management discussed this issue with officials representing the municipal libraries of Puerto Rico, who also were unable to make contact with Ramos or LynxStar. Further, these officials failed to provide any information to support that a fair and open competitive bidding process had occurred. Therefore, the funding request is denied.
