The FCC Form 470 that you have cited as the establishing FCC Form 470 for the FRNs, FCC Form 470# 545770001164948, contains contact information associated with the service provider for the FRN, Caribbean Network (formerly known as Ink Toner Warehouse), Service Provider Identification Number 143037241. This is a competitive bidding violation. Specifically, the following address contained within the FCC Form 470 matches the address associated with Caribbean Network: 76 Shell Castle Club Dr., Humacao, PR. Additionally, it has been determined that the consultant listed on the FCC Form 470, Tamara Diaz-Lupoe, a consultant associated with consulting firm A Piece of Heaven, Consultant Registration Number 16075173, is an officer with Caribbean Network and has a relationship with the designated contact person for Caribbean Network, Allen Lupoe. Finally, it has been determined that the designated contact person for Caribbean Network, Allen Lupoe, is also associated with A Piece of Heaven. The FCC Form 470 must be completed by the entity that will negotiate for eligible products and services with potential service providers. A service provider that participates in the competitive bidding process as a bidder cannot be involved in the preparation or certification of the entity's FCC Form 470.
