FCDL Comment:
MR1: In Block 5 of your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On 08/12/2014, you provided a contract dated 06/28/2014. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to- month services. <><><><><> DR1: FCC Rules require that a contract be signed and dated by the applicant prior to the filing of the FCC Form 471 for the products and services requested. This requirement was not met. <><><><><> MR4: The applicant's request to increase requested amount to $1,200/month could not be processed because the change is not a correctable error under the FCC's Ministerial and Clerical guidelines. According to FCC Order (FCC 11-60), ministerial and clerical errors are defined as follows: "The applicant can amend its forms to correct clerical and ministerial errors on their FCC Forms 470, FCC Form 471 applications, or associated documentation until an FCDL is issued. Such errors include only the kinds of errors that a typist might make when entering data from one list to another, such as mistyping a number, using the wrong name or phone number, failing to enter an item from the source list onto the application, or making an arithmetic error." The applicant failed to supply the documentation that supports requested increase.
