MR1: Per Program rules, closed entities are not eligible for funding. The following closed entities were removed from Block 4 of the FCC Form 471 application; North Yarmouth Memorial School 3931. The student counts associated with the closed entity(ies) have been removed from the application. <><><><><> MR2: The amount of the funding request was changed from $511,467.39 per month to $510,967.39 per month to remove: the ineligible entity North Yarmouth Memorial School. <><><><><> MR3: Based upon review of your FCC Form 471 application, we were not able to validate your requested rural classification for Peaks Island Elementary School. The urban/rural classification is determined by the Metropolitan Statistical Area (MSA) data from the US Census. This entity is located in a census tract that is considered urban according to "Table Of Rural Areas By State." USAC only uses "Table Of Rural Areas By State" in the SLD web site at http://www.universalservice.org/sl/applicants/step04/urban-rural.aspx to determine Rural/Urban classification. The requested discount will be modified to reflect an urban classification. The discount for Peaks Island Elementary School will be modified from 60% to 50%.
