MR1: The category of service was changed from Internet Access to Internal Connections in accordance with Program rules. <><><><><> MR2: The amount of the funding request was changed from $$75,885 to $50,739 to remove: LifeSize Room 220 of $1,189.93, LifeSize Room 220 Maintenance of $846.37, 55" Samsung LED TV of $11,980, LifeSize Team 220 - Phone of $5,849.55, LifeSize Team 220 Maintenance of $1,944.15, Ergetron Neo-Flex Mobile Media Center of $3,180 and MMC Camera Shelf Kit of $156. <><><><><> MR3: Basic Maintenance of Internal Connections FRN was modified from non-recurring charges to recurring charges as required by FCC Rules. <><><><><> DR1: Applicant failed to certify who prepared the responses to the Selective Review Information Request (SRIR) by not completing the SRIR Completion Certification. Therefore, the Administrator cannot verify that the applicant takes full responsibility for the responses. <><><><><> DR2: FCC Rules require that a contract be signed and dated by the applicant prior to the filing of the Form 471 for the products/services requested. This requirement was not met. <><><><><> DR3: During application review, you were asked to demonstrate that you had secured access to the funds needed to pay your portion of the eligible charges. The documentation submitted shows that you did not secure the applicant portion needed for the funding request.
