DR1: FCC Rules require applicants to retain all documentation regarding the competitive bidding process. During application review, you were asked to provide all bids associated with this funding request and you were unable to do so. <><><><><> DR2: FCC rules require applicants to carefully consider all bids and chose the most cost effective solution. Documentation demonstrates that the applicant did not consider all bids. Therefore, the applicant has violated the competitive bidding rules. <><><><><> DR3: The evaluation sheet/bid evaluation matrix was not provided during selective review. Without the evaluation sheet/bid evaluation matrix, USAC is unable to determine that price was the primary factor in the selection of the vendor. Further it is not apparent that the selection of your vendor was in accordance with the criteria stated in your selective review responses, since an evaluation sheet/bid evaluation matrix was not provided <><><><><> DR4: FRN was denied for failure to advise bidders that an RFP was issued. An RFP was issued and the Form 470 advised potential bidders that no RFP existed. <><><><><> DR5: The contract provided during the review was signed on 3/17/14. This Contract Award Date is after the Form 471, Certification Postmark Date of 3/14/14. The rules of this program require that a signed contract with the service provider be in place at the time of the submission of the FCC Form 471 certification. The FRN is denied because your contract was signed after the Form 471 postmark date.
