MR1: The FRN was modified from $9,975.00 per month to $2,886.89 per month to agree with the applicant documentation. <><><><><> MR2: On your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On 3/16/2016, you provided a contract dated 7/30/2012. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to-month services. This FRN has been changed to reference contract number N/A, Contract Award Date of 7/30/2012 and Contract Expiration Date of 6/30/2017. <><><><><> DR1: Applicant has not provided sufficient documentation needed to determine the eligibility of the following item(s): FCC Form 470 that established the competitive bidding process for the services. <><><><><> DR2: A contract for a new service was signed prior to the Allowable Contract Date. The allowable contract date is 28 days after the posting of the FCC Form 470 to the USAC website. Program rules require applicants to wait at least 28 days after posting the FCC Form 470 before selecting a service provider, signing a contract or entering into legally binding agreement, or submitting an FCC Form 471. Since you did not meet this Program requirement, the FRN must be denied.
