MR1: The category of service was changed from Telecom to Internet Access in accordance with program rules. <><><><><> MR2: The shared discount was reduced to a level that could be validated by third party data. <><><><><> MR3: On your FCC Form 471 Block 5 FRN 1624329, you indicated the products/services are being provided on a month-to-month arrangement. You provided contracts dated 5/21/07 and 2/5/2007. USAC modified the FRN to reflect the change from month to month service to contractual service. <><><><><> DR1: The contract award date listed on your contract is 5/21/2007. This contract award date is after the Form 471, Certification Postmark Date of 2/8/07. The rules of this program require that a signed contract with the service provider be in place at the time of the submission of the FCC Form 471 certification. The FRN is denied because your contract was signed after the Form 471 postmark date. The Selective Review team reviewed the additional information you requested the Selective Team to review in lieu the contract originally submitted. The other contract submitted violated program rules. The contract dated 2/5/07 was prior to the required 28-day waiting period computed from the date of the posting of the Form 470 to USAC Web Site. Finally, you stated the service is month-to-month and to ignore both contracts. Each contract provided was in violation of separate program rules and can not be ignored, therefore, your FRN is being denied.
