MR1: The amount of the funding request was changed from $1637/month to $1401.01/month to remove: the ineligible product(s) and service(s) of Additional Listing, Broadband Cost Recovery Fee, Caller ID Name/Number, High Speed IA, Inside Wire Maint Plan, Inside Wire Maintenance, LD Pin/Act Code, No 3rd Number Billing, Non-Telecom Services Surcharge, and Router Equipment Fee. <><><><><> MR2: The category of service was changed from TELECOMM SERVICES to VOICE SERVICES in accordance with Program rules. <><><><><> MR3: According to the FCC Order 14-99, Voice services are subject to a phase down of support beginning in funding year 2015. As a result, the Discount Rate of this FRN was reduced from 80% to 60%.
