MR1: According to the FCC Order 14-99, Voice services are subject to a phase down of support beginning in funding year 2015. As a result, the Discount Rate of this FRN was reduced from 80% to 60%. <><><><><> MR2: In consultation with the applicant, the service provider has been changed to BellSouth Telecommunications, SPIN number 143004824. <><><><><> MR3:On your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On 3/27/2015, you provided a contract dated 11/30/2005. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to-month services. This FRN has been changed to reference contract number 4000-1, Contract Award Date of 11/30/2005 and Contract Expiration Date of 6/30/2018. <><><><><> MR4: The amount of the funding request was changed from $446.19 per month to $403.69 per month to remove: the ineligible service Inside Wire Protection for $42.50.
