MR1: The category of service was changed from Telecommunication Services to Voice Services in accordance with Program Rules. According to the FCC Order 14-99, Voice services are subject to a phase down of support beginning in funding year 2015. As a result, the Discount Rate of this FRN was reduced from 80% to 60%. <><><><><> MR2: The Service Type for FRN Line Item 1 was modified from Miscellaneous to Voice Services to agree with the applicant documentation. <><><><><> MR3: The Type of Connection for FRN Line Item 1 was modified from Installation, Activation and Intitial Configuration to Local Service Only to agree with the applicant documentation. <><><><><> MR4: The Purpose for FRN Line Item 1 was modified from Transport to Voice to agree with the applicant documentation. <><><><><> MR5: The FRN was modified from $2,571.00 per month to $2,904.75 per month to agree with the applicant documentation. <><><><><> MR6: The amount of the funding request was changed from $2,904.75 per month to $2,531 per month to remove: the ineligible services, BL Caller ID for $145.50, Directory Listing for $22.00, Directory Listing-Additional for $26.00, Non-Published Number/Listing for $10.00, BL Call Forward Universal for $18.80, Caller ID for $9.70, BL Hunting for $139.75 and 800 Dir Assistance (Monthly Listing) for $2.00.
