MR1: The Service/Product Type for FRN Line Item # 1 was modified from Internet Access to Voice Services to agree with the applicant documentation. <><><><><> MR2: The category of service was changed from Internet Access to Voice Services in accordance with Program Rules. According to the FCC Order 14-99, Voice services are subject to a phase down of support beginning in funding year 2015. As a result, the Discount Rate of this FRN was reduced from 70% to 50%. <><><><><> MR3: In consultation with the applicant, the service provider has been changed to Cirrus Networks, SPIN 143029560. <><><><><> MR4: The FRN was modified from $5,000.00 monthly to $4,000.00 to agree with the applicant documentation. <><><><><> MR5: The establishing FCC Form 470 Application Number was changed at the request of the applicant.
