DR1:The products and services that were requested on your FCC Form 471 (Dark Fiber Network) are substantially different than those requested on your FCC Form 470 (Internet Access and Transport Bundled). Program rules require applicants to make a bona fide request for services by filing an FCC Form 470. The services requested on your FCC Form 470 must describe the planned service requirements requested on your FCC Form 471. You indicated you posted for the wrong service type. This error must be corrected by another FCC Form 470 with the correct service type. Since you did not meet this Program requirement, the FRN must be denied.||MR1:This is a new FRN. It was created in order to change the service type requested on FRN 1799011141 from and Fiber Maintenance & Operations to Dark Fiber (No Special Construction) in accordance with Program rules.||MR2:On your FCC Form 471, it was indicated the products and services are being provided on a month-to-month arrangement. On 10/11/2017, you provided a contract dated 3/21/2016. Based on this information, USAC has modified the FRN to indicate that these are contractual products and services rather than month-to-month services. This FRN has been changed to reference contractual products and services with a Contract Award Date of 3/21/2016 and Contract Expiration Date of 3/20/2021.||MR3:The establishing FCC Form 470 Application Number was changed at the request of the applicant.||MR4:The FRN was modified from $16975.00/month to $1225.00/month to agree with the applicant documentation.
